Actually, they’re asking what gender were you assigned at birth as. This helps them to know what diseases and sicknesses you are most likely prone to get~! Some of these can be very lethal or serious, however, and so it is much better to tell them the information. It would be okay to write your preferred gender in a little note beside your answer, but please, please do tell them who you were assigned at birth as!

This is the first time I’ve seen someone make this correction without being rude or condescending so thank you friend
^^Holy Hell, I have never seen someone be that polite about this, mostly people are so fucking rude it makes me cringe. But really, for medical reasons, this is important! You can make a note beside it or bring up your gender verbally, but the question of your assigned birth gender can have a lot to do with certain life-threatening conditions, and I’m sorry that you have to do the thing, but I worry for your health!
